Jeffrey H. Lautman, MD

Dr. Lautman has been in practice since 1989. He is Board Certified in Nephrology, a graduate of OSU's prestigious Independent Study Medical School. He completed his Nephrology Fellowship at University Hospitals of Cleveland, Case Medical School. Dr. Lautman is on the forefront of the latest technologies and approaches that help his patients. He was one of the first private practice nephrologists to introduce Home Hemodialysis to Cleveland and the innovative ambulatory 24-hour BP wearable device. Dr. Lautman also pioneered the Chronic Kidney Disease Clinics, Cleveland Clinic Health System, Eastern Region. Recognized for his leadership, he was elected to serve as Chief of Staff for Hillcrest Hospital, after serving as Chief of Nephrology there. He is Medical Director, CDC Euclid Hemodialysis and Medical Director, CDC Euclid Home/Peritoneal Dialysis. He is a past Chief of Medicine at Cleveland Clinic’s Euclid Hospital. For all his titles, Dr. Lautman is most proud of how patients connect with him in a very real and personal way. He loves history, Jewish Bible study/Torah cantillation, the Browns and ballroom dance.

Umesh Yalavarthy, MD

("Dr. Y") ranked in the top three of all Nephrology Fellows in the entire United States in his year. He is Board Certified in Nephrology and Hypertension. He did his Internal Medicine Residency at the University of Tennessee's Medical Center and also served as Chief Resident. He has earned a reputation for going above and beyond for his patients. He brings enthusiasm and sensitivity to everything he does. He is the Medical Director of CDC, Beachwood. He is a past president of the Association of Indian Physicians of Northern Ohio (AIPNO). He was chosen as Distinguished Physician of the Year by AIPNO in 2018, and honored by University Hospitals in 2019. Physician-colleagues hold him in their highest regard; patients love him; you will too! In his spare time Dr. Y loves volunteering with his family and reading.

Sheru Kansal, MD

Dr. Sheru Kansal is an accomplished nephrologist practicing in Cleveland, Ohio. Dr. Kansal received his medical degree from University of Buffalo, where he met his wife, who is also a physician (radiologist) in Cleveland! Dr. Kansal is recognized for his exceptional communication skills, ensuring patients are actively involved in their treatment decisions. His commitment to excellence provides his patients with the most up-to-date treatments of chronic kidney disease (CKD), hypertension and ESRD (dialysis). He is well-versed in the intersection of diabetes, congestive heart failure, hypertension and other diseases that affect the kidney. He emphasizes preventive care to slow kidney decline and promote optimal kidney health. Patients trust Dr. Kansal for his expertise, compassion, and dedication to their well-being. In his free time, Dr. Kansal enjoys the outdoors with his wife and three beautiful girls. Skiing and hiking on the mountain is where you will typically find them. His other interests include cooking fabulous meals for his family and tinkering around the house. Sometimes, he even fixes something!

Lisa M., MSN, CNP

Lisa is a focused and energetic Nurse Practitioner. Her past experience in the Cleveland Clinic's ICU is an ideal background for a nephrology NP. Her bright smile and sharp mind is a powerful combination. You can look forward to seeing her collaborating with your CKHC doctor and others on your medical team, at the hospital or at the dialysis center, with a holistic approach to your care.

Lynda N., APRN-CNP

Lynda began her distinguished career as a Clinical Nurse Specialist and then as a Nurse Practitioner. She is a graduate of Kent State University and Case Western Reserve University. Lynda started the peritoneal dialysis program at University Hospitals and later added home hemodialysis and CKD and anemia management to the unit’s offerings. She has published many articles related to CKD and has been a frequent speaker at local and national conferences. She comes to us from the Cleveland Clinic’s Hillcrest Chronic Care Clinic, bringing a wealth of knowledge about the management of anemia and mineral bone disease.

Tim R., DNP, CNP, CNN-NP

We are fortunate to have had Tim with our practice since 1999. We were delighted to celebrate Tim's 20th year with our practice in June 2019! He has both a Masters and Nursing Doctorate from Case Western Reserve University (CWRU). He is a Board Member of the CNN-NP and Test Writing Committee, NNCC; and on the clinical faculty at CWRU, Kent State University and Ursuline College. He is a published author on nephrology. Tim is a natural leader. He founded and coordinates a professional group of nephrology nurse practitioners in the greater Cleveland area. In addition, Tim works closely with various patient-centered kidney organizations locally and nationally, such as the NKF, ANNA, and The Renal Network. He brings to bear a profound knowledge of nephrology and hypertension to each patient under his care.

Chief Administrative Officer

Teri has supported the phenomenal growth of our practice, coordinating all of our staff and services, and introducing new technologies that allow us to serve you in the most up to date methods. For over 20 years she has worked behind the scenes to ensure that we provide the best service to you. For fun, Teri likes to ballroom dance with her husband, travel, and visit her kids and grandkids.

Assistant Manager

Jan has been with the practice for 30 years, since Dr. Lautman was a solo practitioner! She oversees multiple processes in our office and handles all issues with sensitivity.

Lauren, LPN
Clinical Supervisor

Lauren has always had an interest in helping others. After seven years as a nursing assistant, she pursued her dream of becoming a nurse. She graduated at the top of her class from Willoughby-Eastlake School of Practical Nursing and worked for four years in primary care at Lake Health. Now at CKHC Lauren handles several responsibilities as Clinical Supervisor. She works closely with Dr. Lautman before, during and after your visits to ensure you're provided with an excellent care from our practice. Clinical Secretary

Amy is a people-person! She has a calm and joyful way about her. You'll see Amy at the front desk with a warm smile, ready to check you in and make you feel at ease when you arrive. A couple of days before your appointment you may get a gentle reminder call from her. Amy graduated from Kent State University with a Master's degree and in her free time she enjoys taking her two vizslas kayaking. Courtney
Medical Assistant

Courtney has one of the biggest smiles and personalities! She started off as a young girl with a passion for always wanting to help and care for others. At the age of 17 she became an STNA, and several years later also became a private caregiver. In 2017 she attended Remington College where she received her diploma in Medical Assisting with X-ray (limited scope). Right after graduation she was hired with Visiting Physicians Association as the lead medical assistant specializing in home based primary care. Courtney's journey with CKHC started in August of 2024 as a medical assistant working with nephrologist Dr. Yalavarthy and Dr. Kansal. Courtney enjoys being a part of the CKHC family as well as being able to care for patients and their needs. In her free time she is still a caregiver, a mother of two beautiful children. Gabbey has been a RCMA for 13+ years now, graduated with honors and top of her class from Kent. With a wide variety of knowledge and passion in health care, she has now found her forever home with CKHC! Gabbey is Dr.Kansal’s medical assistant and follows alongside him at our 3 locations. In her free time she loves spending time with her 2 children and family, competing in spartan races, travel and anything outdoors like camping and fishing.
